To register for an Ayushman Bharat card, you can visit an empanelled hospital or Ayushman Mitra and provide your identification documents, such as your Aadhaar card. 

Individuals from the SC/ST category, households lacking a member aged above 16 earning income exceeding Rs. 2.5 lakh, and those included in the PMJAY beneficiary roster are eligible to enroll in the Ayushman Yojana.
